Creating a Referral Template

  1. Go to Administration > Center Management > Referral Templates
  2. Click + New Template.
  3. Type a name for this Referral Template.
  4. Type and select the user who will follow up by default with referrals that use this template.
  5. Type and select the users who will be notified by default for referrals that use this template.

Accudemia provides a sample questionnaire as part of the default template. Customize it to your needs.

Creating a Referral

  • Once there is at least one referral template, new referrals can be created by navigating to: Campus > Referrals

To Create a Referral

  1. Click on + New Referral.
  2. Select the Referral Template the referrer should utilize.
  3. Type and select the instructor.
  4. Type and select the class.
  5. Type and select the student.
  6. Type and select the user or users who will follow up with the student.
  7. Select a date to follow up on the referral. This is used as a reminder of when this referral should be reviewed/updated.
  8. Type comments about the reason for the referral.
  9. Choose whether the student receives the referral updates.
  10. Type and select the users who are going to be notified about updates to the referral.

Questionnaire data

  1. Complete the Referral Questionnaire.
  2. Click the Save Changes button.
Follow up By can be left blank and then edited using the Edit button.
