Referral Templates
Purpose: This section is used to show how to create templates so that tutors and instructors can create referralswith their students.
How to Access: Administration > Advanced > Referral Templates
Create Referral Template
Click on the + New Template button. A pop-up menu should appear and you will then be able to see the following:
- The first section is the General Information section which includes:
- Name - Name of the Referral Template you want to add.
- Instructions - Beginning of the term you want to add.
- Follow up by - End of the term you want to add.
- The second section is the Questions section which allows you to:
- Edit Page Title - Set the Referral title for the page (What the User will see at the top of the questionnaire).
- Edit Page Description - Setting a description is optional but can help describe what the referral is for or provide instructions on completing
- Edit Question - Allows you to write the question that you want the users to see; there are many customization tools available to use.
- Select Response type - The following are the three available response types:
- Open Text - The response will be text entered by the user completing the Survey.
- Multiple Choice - Where you can define Answer choices for this Question.
NOTE: When you select multiple-choice, a new check box will be displayed with the option to allow multiple responses. - Label - Where you can add additional text to help explain a question or provide additional help to complete the survey.
- Select if Answer Required - Allows you to decide whether or not the question is required to be answered. Checking the checkbox means it's required.
- Add Question(s) - Creates a new question when the Add Question button is clicked
Once finished click the Save Changes to continue.
